Terms of Usage
In order for us to enroll you in TimeWise Medical and to see you as a patient, you must read and agree to the following conditions:
- I will pay my bill at the time of service with a valid credit card (VISA, MC, or Discover).
- I understand that TimeWise Medical will not bill my insurance company.
- I understand that TimeWise Medical will not pay for interpreters.
- I understand that TimeWise Medical will not pay for any tests which are done off-site. Most of these tests will be ordered through Fairview Ridges Hospital.
- It will be my responsibility to either pay for that test or to have the bill processed by my insurance company.
- I agree to pay the Minnesota Provider Tax (set at 2% as of May 1, 2009) which will be added to my bill. (This is a state imposed legal requirement and we cannot waive that charge).
- Disputes: I agree to mediate, with the help of a professional mediator, any and all disputes arising from my experience at TWM. If mediation fails, I understand that I can still initiate a lawsuit.
- I understand that it may take 24 hours or longer to respond to emails. TimeWise Medical will strive to return emails in a timely manner. Phone calls are returned more quickly than emails.
- Email: The TimeWise email system is secure. Only the physician has access to it. Your personal email account may or may not be secure. If others have access to your user name and password, they could read this and all other email you receive. For this reason, be careful with email messages to TimeWise Medical. Personal issues and details are best discussed in the office setting.
- Telephone Calls: I agree to pay for any emails and telephone calls which take place outside of normal office hours. See the homepage tab for current charges for phone calls.
- I agree to be on time for appointments and to give reasonable notice if I will be late or if I must reschedule.
